Passport File Search Request
The passport file search request is used for applicants applying for a lost passport replacement and the applicant does not have a certified copy or original birth certificate available. It can only be used for lost or stolen passport applicants. This request will cost $150 and paid with a check to the US Department of State.
Applicants that have enough time may prefer to replace their missing birth certificate. You may also order an express birth certificate. The long form birth certificate is a requirement for your passport application unless you have a valid adult passport that needs to be renewed. The birth certificate is used to prove citizenship. The only other option is if the applicant once had a passport and it is in the U.S. State Department files. They charge $150 for the service to look up your lost or stolen passport and to use that as the proof of citizenship.
